West Yorkshire Integrated Care Board has chosen IEG4 to deploy a comprehensive digital continuing healthcare (CHC) platform at Bradford District and Craven Health and Care Partnership (HCP).

The company said the selection is designed to streamline the All Age Continuing Care (AACC) process throughout Bradford and improve the department’s operational capabilities.

The platform integrates multiple processes into one digital system, covering workflows for staff, the management of the entire assessment and referral process, and case management for patients who are eligible for AACC and S117 aftercare.

The Bradford and Craven system will also feature a portal that provides users with real time updates on progress, along with 24/7 access to administrative and payment tools when necessary.

Vickie Milner, Head of AACC Bradford at West Yorkshire ICB said the choice was based on the system providing the most efficient, end-to-end approach available.
