OneLondon has said the Universal Care Plan (UCP) for patients in the capital is to provide information on more conditions from next month.
This will cover long term conditions including dementia, frailty and learning disabilities.
The move follows a consultation with health and care professions who wanted a more flexible digital care plan to support patients with different health needs.
The UCP is an NHS service that digitally shares personalised care plans with health and care professionals across London. It was initially set up July 2022 to support end-of-life and palliative care planning, and extended this year to support people sickle cell diseases.
It currently holds over 60,000 care plans with around 25,000 views a month by health and care staff.

OneLondon – a group of integrated care systems leading the work on the development of the UCP – has developed a learning page with a quick reference guide outlining the contents of each form in the care plan, and a training portal and 10-minute video.
Nick Tigere, head of the UCP programme, said: “This is a very exciting time for the Universal Care Plan programme as we prepare to expand the tool into a broader personalised care and support plan.
