Technology has a major part to play in integrating and simplifying the patient journey between hospital and home - improving lives and freeing up vital resources at the same time. NHS Digital and NHS England have recently put the focus on improving delayed discharges with a £1.4m pot of funding for local government to work with NHS colleagues.
UKAuthority's Helen Olsen Bedford joined NHS Digital, the LGA, Essex County Council and Microsoft in the studio to discuss how to get the patient journey right between health and social care - and how best to then keep people safe and well in their own homes for longer:
