Bath and North East Somerset, Swindon and Wiltshire Clinical Commissioning Group has laid plans to extend its integrated care record to cover its whole area.
The CCG has awarded a four-year contract to Graphnet to provide its CareCentric software, which had already been contracted for the Bath and North East Somerset area, into Swindon and Wiltshire.
This follows the merger of the CCGs for the three areas in April of last year.
A spokesperson said the implementation has now begun and is expected to last into next year. It is a complex deployment that involves joining up information on 934,000 residents from more than 100 health and social care organisations.
It will involve 94 general practices, Great Western Hospitals NHS Foundation Trust, Salisbury NHS Foundation Trust, Wiltshire Health and Care, Royal United Hospitals Bath NHS Foundation Trust, Swindon Borough Council, Wiltshire Council, Bath and North East Somerset Council, Medvivo, Virgin Care, and Avon and Wiltshire Mental Health Partnership NHS Trust.
Staff will be able to access information on individuals from their local systems using a single sign-on.
Jason Young, assistant director of digital transformation at the CCG said: “Health and care professionals who work across the BSW area will soon have easier access to the information they need to care for those who use our services. This will support improvements to care and help our services be as efficient as possible.”
The CCG also plans to use the platform to understand the region’s health and care needs.
