Staffordshire and Stoke-on-Trent STP has awarded Graphnet a contract to develop an integrated care record for the region.
The sustainability and transformation partnership has signed for the use of the company’s CareCentric system to link up its 151 GP practices, two acute trusts, three community and mental health trusts, two local authorities and six clinical commissioning groups.
The shared record will be known as One Health and Care and enables professionals from the organisations to view and contribute information relevant to people under their care.
It will also enable individuals to view their personal records, and include intelligence and report capabilities, enabling the STP to analyse the care record data.
Simon Whitehouse, director of the area’s Together We’re Better programme, said: “Local people have told us that they want their health and care records to be available to practitioners who care for them, which means that they only need to share their health history once.
“By connecting these pockets of information, it will help us to be more efficient, so we can improve quality and outcomes for those being treated and cared for across both health and care.”
