The Northamptonshire Health and Care Partnership (NHCP) has agreed on a contract with Graphnet Health to develop a shared care record for the county.
It will bring together information from the whole care community, involving Northamptonshire County Council Social Care, 69 GP practices, their out-of-hours service providers, Northampton General Hospital NHS Trust, Kettering General Hospital NHS Foundation Trust, Northamptonshire Healthcare NHS Foundation Trust and the East Midlands Ambulance Service NHS Trust.
The record will be integrated into existing the organisations’ existing IT systems and accessed through a single sign-on and in patient context. It will draw on GraphNet’s CareCentric software to support assessments, data collection, workflow and notifications.
The contract is set to run for three years, with a possible extension for another two, and while project plans are still being prepared NHCP expects the system to go live late this year.

“We are delighted to be making progress on delivery of this programme”, said Kirstie Watson, digital lead for NHCP.
“The benefits will be far reaching. For the care community, it will enhance the quality of the information available so we can provide faster, safer care.
“It also means that authorised clinicians and carers will have the right information where and when it is needed, which will in turn make their jobs easier. The time they save looking for a patient’s information can be used to focus on delivering more care to our citizens.”
Further phases of the project will include the analysis of data from across the county, so care providers can better understand and manage the population’s health and care needs, and also target interventions at relevant individuals or groups.
