Lincolnshire Community Health Services NHS Trust is planning to launch an online system for handling patient referrals.
It has been working on the project with digital health company Inhealthcare and plans for the system to go live early in the new year.
The company is currently creating referral forms for 10 services including diabetes, tissue viability, heart failure, Macmillan support, Parkinson’s disease and transitional care.
It said the system will reduce the time staff in the trust’s operations centre spend on fielding calls for other providers or following up information on incomplete referrals, and allow them to focus on their primary roles of guiding patients to the correct community health services.
Tracy Pilcher, director of nursing, allied health professions and operations at the trust, said: “This new system will enhance the patient experience and improve the quality of service we provide.”
